I Paula Rist (Nurtured Mind) is committed to upholding the highest possible standards of professional ethics and codes of practice in the interests of client care and confidentiality.

The following is an agreement between Paula Rist (Nurtured Mind) and You the (Client), who will endeavour to ensure the following terms will be carried out.

Confidentiality is vital and will be maintained in all but the most exceptional circumstances. I agree that these can include: Legal action (criminal or civil court cases where a court order is made demanding disclosure, including coroner’s courts), child abuse (if I am an imminent danger to myself or others and where there is good cause to believe that not to disclose, would cause danger or serious harm to others).

I have been advised the scope of therapy being provided by Paula Rist (Nurtured Mind) and I give my full consent to receiving therapy sessions. I understand that results vary from person to person and the agreement between myself and Paula Rist (Nurtured Mind) will be to work on any issues or problems presented to me using whatever model, knowledge or technique appropriate to my situation.

Services provided to me are designed jointly with myself and Paula Rist (Nurtured Mind), dependent on the nature of the service required. Every effort will be made by Paula Rist (Nurtured Mind) to provide the very best service she knows, tailored to me. However, it no way implies or guarantees a ‘cure’ of the said issues or problems.

Services may range as appropriate to my needs and may include the following: Mindfulness, yoga, hypnotherapy, NLP, EFT and CBT. All of which are of the highest standard and certification in helping me with my needs.

Any session recording or support material provided, is provided at Paula Rist’s (Nurtured Minds) discretion. Throughout Paula Rist’s (Nurtured Mind) and my relationship, Paula Rist (Nurtured Mind) will engage in direct and personal conversation and will keep all information provided strictly and confidential. I can count on Paula Rist (Nurtured Mind) to be honest and straightforward in asking questions and making requests. I understand that the power of the relationship can only be granted by myself and I will communicate to Paula Rist (Nurtured Mind) if the relationship is not working as desired, and can terminate any or all of the sessions at any time.

I understand that (Mindfulness, Yoga, Hypnotherapy, NLP, EFT and CBT) or any other therapy provided by Paula Rist (Nurtured Mind) is not a replacement for medical treatment, psychological or psychiatric services or the appropriate counselling. I also understand that Paula Rist (Nurtured Mind) does not treat, prescribe for, or diagnose any condition. I declare that if advised prior to any session with Paula Rist (Nurtured Mind) to seek medical approval, I will consult my general practitioner and/or hospital consultant and gain the appropriate medical approval for working with Paula Rist (Nurtured Mind).

Please note that payment for sessions will be made in advance and that the full fee will be charged if cancellations are made in less than 24 hours, prior to the booked session.
